Child Safety Registration-Global Gates Gatherings

Thank you for participating with Global Gates Gathering at First Orlando. All participants and volunteers 17 years and older must 1) register using this form. 2) complete MinistrySafe’s ‘Sexual Abuse Awareness Training’, 3) read/sign the Global Gates Child Safety policy, and 4) read and sign the Indemnity and Release Form in order to participate. Please complete the information below after you complete step 2 which requires for you to upload a completion certificate. Contact safety@globalgates.info if you have any questions.

A background check is required and will be ordered for you. Look for an email from Checkr sent to the email address you provided. Please respond to the email as soon as possible. You will be giving them consent for the background check.

If you are a minor 17 years old and under, one of your parents must sign the Indemnity and Release Form.

Step 2: Ministry Safe

You will receive a link from MinistrySafe to take the Sexual Abuse Awareness Training. Once you take the training, you will upload the completion certificate to this form.

Step 3: Global Gates Child Safety Policy

Please read the Global Gates Child Safety Policy and sign and date that you are in agreement.

I have received and read a copy of Global Gates’s Child Safety Policy and understand the importance of the material.

I understand that any violation of these policies will result in disciplinary action up to and including termination of employment or ending the volunteer relationship.

I, the undersigned, fully recognizing the dangers and hazards inherent in the Global Gates Sifting Weeks/Volunteer Trips, and any related transportation, including personal injury, property damage, or wrongful death, as well as the unknown dangers and hazards which may arise in the course of my participation in the Global Gates Sifting Weeks/Volunteer Trips, do hereby voluntarily:
Agree, for myself, my heirs and my personal representative, to defend, hold harmless, indemnify, release and forever discharge, to the broadest extent allowed by law, Global Gates Network Inc., its trustees, officers, employees, agents, insurers, successors, assigns, from and against any and all claims, demands, actions, or causes of action on account of any damage to real or personal property or any personal injury or death that may result from my participation in the above Global Gates Sifting Weeks/Volunteer Trips.
